An itemized statement is a periodic document issued by a financial institution, such as a bank or brokerage firm, to its customers detailing all account activity for the period. Itemized statements include deposits, credits, debts, fees and all other pertinent activity. Usually, this information is presented in chronological order, although it may be broken down in several different ways for the customer’s convenience.

Description:

An itemized receipt must contain the provider name, type of service, date of service, and cost of service. For health care expenses, a copy of your Explanation of Benefits (EOB) typically contains all the required information. You can generally access this information from your health plan login site if a copy is not automatically mailed to you. Alternatively, you can request a detailed statement from your health care provider. For prescription expenses, an itemized receipt is typically included with your drug information details. The itemized receipt will include the drug name. A standard credit card receipt doesn’t typically provide the detail required to verify the expense.

An itemized bill is a detailed billing ledger that a provider can give you, which lists all of the services you received during the visit, and includes the procedure (CPT) and diagnosis (ICD-10) codes. It differs from a patient billing statement, which only includes the patient balance, and does not include a description of charges or any codes. An itemized bill is like the receipt you receive at the grocery store, which enlists all of the items you have purchased, whereas a patient statement is like a credit card receipt, which only shows the amount you owe. A client can request a copy of your itemized bills by calling your provider’s billing office and asking for an itemized ledger of the services you received. This should be provided to you free of charge and can be sent to you via email, mail, or fax. Be sure to request that you receive an itemized bill, and not just a billing statement so that you can review the procedure (CPT) and diagnosis (ICD-10) codes and a description of services.
